Proctoring for DRC Exams

As of Fall 2007, the SHAC Testing Office tests all students who receive accommodations through the Disability Resource Center.  You will no longer receive a red envelope with a proctoring form inside.  You can download a proctoring form from this website or pick one up in our office.

The SHAC Testing Office is an option for instructors/students to use to assist faculty in providing the required testing accommodations indicated on the students "Approved Academic Accommodations" Form. It is not a requirement that students/faculty use our services. Faculty may opt to provide accommodations. The SHAC Testing Office Hours:

Monday - Friday 8:00 AM to 5:00 PM. All exam proctoring will be done within these hours of operation.

Instructor:

  • Fill out a DRC Proctoring Form. (Click on one of the links below to download form.) Save it to your desktop and print out to attach or send as an attachment with your e-mail.

  • Deliver or send the proctoring form and test materials to Testing Services. Materials may be hand-delivered to 310 UCB, faxed to 503-725-2551, or sent via e-mail to testing@pdx.edu. Proctoring forms are REQUIRED.
  • Test materials will be kept secure. Student will call to make an appointment prior to the deadline the faculty has set.

  • Testing staff will administer and proctor the test according to your directions and deadline set.

  • You may pick up the test at our office, it can be returned through campus mail (not recommended), or the answer sheet can be scanned and emailed to your email address.
  • It is the student's responsibility to make appointments with our office, not the instructors.

  • Tests will NOT be administered past the deadline without your permission.

Student: 

  • It is your responsibility to visit the DRC office and complete the Service Request Form at the beginning of each term. You will list your schedule of classes and the accommodations that have been authorized for you by the DRC office.

  • Present your instructor with the Accommodation Letter prepared for you by the DRC at the beginning of the term and discuss the testing process and your particular accommodations needed with your instructor.

  • Remind your instructor (5) days prior to your exam that you plan to use testing accommodations.

  • Request your instructor go online and download a DRC Proctoring Form from our website at: http://testing.pdx.edu , attach it to the exam, and email it to: testing@pdx.edu, deliver it or fax it (503.725.2551) to the Testing Office well in advance of the exam time.

  • It is your responsibility to contact our office to make an appointment 5 days in advance.

  • Check with our office in advance to make sure the exam has arrived.

  • Students MUST BE ON TIME THE DAY THE EXAM IS SCHEDULED!

  • Students should be ready to take the exam at the scheduled time.

  • IF ACADEMIC DISHONESTY IS DETECTED DURING THE EXAM, THE EXAM WILL BE STOPPED AND RETURNED TO THE INSTRUCTOR IMMEDIATELY.

Rescheduling An Exam

Exams may not be rescheduled without the instructor approval. If, for any reason, you cannot take the test at the scheduled time (i.e. illness) you will need to first: notify your instructor as early as possible; second: contact SHAC Testing Office to cancel your appointment. You are responsible for rescheduling your test.
